Should we do anything in particular if the Patient has a chronic condition that needs monitoring?

For all Patients who have chronic conditions that need continuous monitoring, let them know that we can offer continuous monitoring for them. Set up a care plan and work closely with them. The Patient care manager will help get it organized, but the provider needs to communicate this to the Patient during the visit.
